Incident report form

The online report form can be used by anyone – instructors, students, parents – to report an incident. Once an incident has been reported, the Lead Instructor and Safeguarding will review immediately and will handle as required. 

Users of this form can submit information anonymously if required. No personal information is recorded unless submitted by the user.
